camper forms
Camper Forms are now available!
You can login into your CampInTouch account at any time to check the status of your account, make sure your contact information is up-to-date, and to complete the pre-summer forms.
All forms necessary for your camper(s) will be listed on your Forms Dashboard within your account. Forms in BOLD at the top are required, while forms in regular font are optional. Please note that if you stop in the middle of completing one of the forms, what you have already entered will NOT be saved. When a form is completed, it will turn green and move to the bottom of the list. Medical forms that must be reviewed by our staff will remain in the list and say “Uploaded” with the date until approved. Once you have submitted a form, you can review the information that you entered but you cannot edit it. Please contact our office if you need to make any changes.
Required forms are due by May 1. Optional forms are due by June 1.
Scrolling down to the bottom of the page, you will see “Forms & Documents for Your Family” which includes helpful informational documents for you to review but do not require completion or signature. Our Parent Handbook is also available in your CampInTouch account under “Documents”. This has every single bit of information you need to prepare for a fantastic summer at 6 Points!
To ensure the best possible care of our campers and to deliver a safe and secure camp program, it is imperative that each family complete and submit all necessary forms on time. Please understand that there is quite a bit of processing required of our office and medical staff, and it is vital that all steps are completed long before camp begins. If for any reason you’re having trouble with your forms or your forms cannot be submitted by the provided deadline, please reach out to our office. Campers without the full complement of forms may not be admitted to camp.

Health and Medical Forms
Must be completed for each enrolled child.The Health Forms are a combination of online forms as well as forms you will need to download, print, complete and upload back to your account. You will receive emails with specific instructions leading up to the summer.

Optional Forms
B’nai Mitzvah Form: For campers who will be celebrating their Bar/Bat Mitzvah shortly after camp and would like to continue with tutoring while at camp.
Roommate Request Form: Rooming is done on the basis of school grade and gender, and requests must be mutual. Camp makes every effort to fulfill requests but reserves the right to make all roommate assignments.
